Description: This patient presented with mild breast droopiness and desired fuller breasts without a breast lift. Dr. Maryam Zamani selected 415cc Sientra HSC+ highly cohesive gel implants in a high profile and placed them in the subfascial plane above the muscle using a preservation breast augmentation technique. For select patients with mild ptosis, over-the-muscle implant placement can allow the implant and native breast tissue to move together as a single unit, creating a more natural appearance. In contrast, under-the-muscle implants may occasionally contribute to a “waterfall deformity” or “double bubble” appearance, where the implant remains elevated by the pectoralis muscle while the natural breast tissue settles lower on the chest. By placing the implant above the muscle, Dr. Zamani was able to enhance volume, improve breast shape, and create a more harmonious relationship between the implant and the patient’s existing breast tissue. Her results demonstrate fuller breasts, improved upper pole fullness, and natural breast contours while preserving pectoralis muscle function and avoiding animation deformity.
